Are you looking at the midterm written response or the final multiple choice? Can you share a screenshot of the settings if the midterm? If the quiz, the student's grade is overridden, click on the name of the quiz to get to single view and uncheck the overridden checkbox.

You might look at setting up Wordpress on the front end, using Woo Commerce which would give a lot more flexibility and using Edwiser Bridge to link the two. That adds a whole other site though...
Did you restart php after switching the input vars? It won't pick up the change until you do that. Also, make sure you are changing it in the correct php version. My suggestion would be to use the latest 4.2 code on the migration - as Howard said, if it fails it is just an annoyance. Just make sure you keep copies of the original db and files in case you have to restore to that. However, if you also bring over the moodle code folder, then you will be at exactly the point of your existing server and that would take care of the migration and then you can jump. Good idea to jump to latest 4.2 first and then moving to 4.5.
Weird - try setting AccessAllGroups to Not Allow and see if that fixes it.
