Oh Rick knows more than I do as I have never even worked with D2L. I did do a quick forum search on here for D2L and it is not very promising - I found people with similar problems as yours but nobody with a successful result - I concur with Rick that it might be easier to recreate the courses. A lot depends on the type of materials in the D2L course.

What is the error? How are you uploading them? I haven't personally done this but there is documentation here IMS Common Cartridge import and export

So in admin/settings.php?section=qdefaultsetting ? Currently it states "These defaults are used when a user first previews a question in the question bank. Once a user has previewed a question, their personal preferences are stored as user preferences."
Or are you saying that such a clarification should be added - and perhaps improved - also to admin/settings.php?section=qtypesettingmultichoice ?

I think you can hide it - a little bit - by disabling the Question history plugin though from Site admin > Plugins > Question bank plugins? You won't see it on the question bank page then but if you edit a question it will still show which version it is as you edit it.

When you say "Orange demo" do you mean https://school.moodledemo.net/? As a manager you can play with the (now standard in LMS) custom reports. Log in with username manager and password moodle.
The site doesn't have or allow any plugins however.
