Thanks! I have restored your questionnaire to my Moodle test site running:
Moodle 3.11.4 (Build: 20211108)
Questionnaire 3.10.1 (Build - 2021080400)
and I can report that everything is working OK. I do not see the problem you reported. Maybe you should try to upgrade to a more recent version of Questionnaire?

Actually, it's not "export" but "backup" (and "restore"), see the screenshot below.

When you are on the "backup" pages, you'll have to un-check all of the options, except: Include activities and resources.
Then go to the Activity backup area page, find the backup you have just done and Download it to your computer. Then attach that file to your reply on this forum. The backup file will look something like: backup-moodle2-activity-109-questionnaire109-20211119-1540-nu-nf.mbz

It's impossible to find an answer to your problem without seeing the exact contents of your questionnaire. Please export your questionnaire and attach it to your next post so I can examine it and hopefully find a solution.

That seems a rather convoluted way of creating H5P activities in Moodle.
Why don't you create H5P activities directly in your Moodle course Content bank? When you insert such activities in your course you have a whole range of options available. You cannot restrict the number of attempts at H5P activities, but you can decide how to grade them.

IMHO H5P activities are not suitable for testing the student's knowledge, but for practising. This is why rather than restrict their use you should encourage re-trying as often as the student wants.
