There's a bug on this:
http://bugs.moodle.com/bug.php?op=show&bugid=86
It's not a trivial change because sometimes pages are in Moodle frames and sometimes not.

It sounds like you need to configure Apache to recognise index.php as a default page:
http://moodle.com/doc/install.html#webserver

Yes, editing footer.html is the best way to do that for now, and yes this forum or the admin forum is probably best.
I would suggest making a copy of a theme folder with your own name and editing that instead - that way it won't get overwritten when upgrading.

1. Yes, notifications gets sent after the same period as forum posts (eg 30 minutes). And yes, if you re-edit AFTER that period then notifications get resent.
2. When you look at assignment submissions, all the assignments that need grading are coloured differently (darker). eg one that has been resubmitted. Grades are not wiped until you change them.
3. No way to insert a whole topic yet. Shuffle time.
Cheers,
Martin
