Questionnaire (plugin)

Hi

I'm having difficulty with the the questionnaire module in v 1.6 I can create the questionnaire and add questions but I am unable to save it. I get the error message "Could not add a new instance of questionnaire", but it does appear in the database.

I've tried firefox and IE, with no luck. Any advice much appreciated
