Hi
I seem to be only able to have one choice per page.. How can you have many choices on the same page. For example I would like my page to look like the survey page with lots of questions and multiple choice answers..

The choice module is setup for just one question (multiple choices) per page. You may want to check out the Feedback module or the Questionnaire module. They will do what you want. Neither one are in the standard install, so you might look at the Modules and Plugins area of the main moodle.org site.
I've used both. Feedback does everything I need and it seems a little easier to get a survey up and going.
