No retake option

I have 1 quiz that a student didn't get a passing grade on, and they don't see the option to retake? When I click edit, editing scorm package, i show number of attempts as "unlimited" 

Anything else I can check to see why the student is unable to retake quiz

Do you have it skip content structure? Is preview mode on or off? Those are appearance settings.

You might change Disable preview to yes and see if that helps. Preview mode does weird stuff if you want to do retakes.

Is it actually reporting completion? Some modules report passed/failed. Others just complete/incomplete. If your module says incomplete, it won't trigger a retake because Moodle thinks they have not finished. If it does that and you have a passing score, you could set a required passing score in moodle and set the module to just report they completed (but Moodle knows they didn't pass). I don't know what you're using to publish the module but Captivate lets you choose which type you want. Lectora allows you to set a score and to set complete/incomplete.

Take a look at what is actually being reported.

I see this if I go to the quiz report, under learning objects for the student:



The 'require grade' option is greyed out bc completion options is locked:

This activity has already been marked as completed for 9 participant(s). Changing completion options will erase their completion state and may cause confusion. Thus the options have been locked and should not be unlocked unless absolutely necessary.

If I choose to unlock, will it cause big issues?

Unlocking will remove all learners current completions -
What does the Course Activity Report and Course Completion Reports display? Are all checkboxes selected for the students that are complete? Are the student's with the incomplete status marked incomplete for the SCORM activity? Is the quiz inside of the SCORM activity or are you using Moodle quiz?

Course Administration>Reports>Activity completion
and
Course Administration>Reports>Course completion

Thank you, never looked there:

I see activity completion, and for the student, it does NOT have a check mark for the quiz.

Under reports, I don't see a course completion, I just show it under the course admin section:

Not having a Course completion report would indicate that it may not be activated in site's Moodle.
Site Admin>Courses>Course default settings
Scroll down to Completion track = Yes

Consider reviewing your Activity completion conditions - 
1. Are they defined?
2. Does their definition match how the SCORM package was created? (Pass/Fail or Complete/Incomplete or number/% of slides viewed)

For the SCORM activity or Quiz activity - do you have Activity completion conditions?
