Grades not showing up, or disappearing after taking quiz

Having an issue where students are taking quizzes, get graded, but on the course itself the grades are not showing up; or they show up then later on when looking at it again, they disappear? 

I see this under the course:



But if I click on the grades icon on the left menu, I just get this:

Look at your Gradebook items and Category - are they hidden?

I see this, under : Dashboard/ Site administration / Courses / Course default settings

Show gradebook to students
moodlecourse | showgrades -> YES

"Hidden" is not enabled. If I click on the activity itself I see grades, but not some users it doesn't show up in general grade book.

I believe that is the Grades Report vs. the Gradebook Setup. Course Administration>Gradebook setup.

When I look at grades / grade history, I see under the Source field "userdelete" what is that in reference to?

I would review the Quiz Logs . You can Filter the All Source = Delete -
Hopefully, it will display a Quiz attempt deleted record and user ID who performed the delete action.

Another symptom, when I look at participants, I see this for students, even though they have a quiz grade when they click on the quiz directly:



When the student clicks on the quiz:



Not sure if thats how being tied to LDAP works, but seems like when it reloads the student into the course; it messes with showing the grade? Just a thought.

Are you sure they finished and submitted the quiz? These could be scores from unfinished attempts maybe?

Yes, have verified that the quiz was submitted.

Currently only 'workaround' i've found was, click overridden check box, save, uncheck overridden checkbox, save.

Grader report -> controls > gear/edit icon -> click hidden, save -> unclick hidden, save

This appears to only be temporary, grades disappear after a few mins.

I'll add that the issues appear in quizzes based on SCORM packages, not sure if that makes a difference or not.

I also tested with a non-scorm quiz; (just added a generic quiz to a course via "add an activity or resource") and seeing same results. But now I'm wondering if it has something to do with LDAP related courses? I tested the moodle quiz feature on 2 courses, one that is tied to my AD, and one that is not. The one tied to AD, I see that userdelete under source in the gradebook grade history report. However, if I go to the course not tied to AD, it just shows as source is system or mod/quiz:

Okay, yes, I added the non AD course into AD and now it shows the same symptoms. Maybe an issue with the cron job? I took course back out of AD and now the grades don't disappear.

We use AD for auto enrolment.

Are the students actually showing up as enrolled as students? Participants->Does student show in their role?

Because if you see this with quizzes and scorms, that's not a scorm problem. Maybe your AD sync is weird.

It seems like if I do manage to get the grades to show. Say if I try to check/uncheck hidden or overridden boxes the grades show up. But as soon as the student logs in and tries to view the grade, it reverts back to '-' in the grade book

I noticed that if I go to course, then grades. If I click 'turn editing on'; if I click on the gear icon for the whole course (gradebook setup / edit category)

I keep seeing hidden checked:


Each time I uncheck it and save; if I go back to look at it that box is rechecked again. How do I keep it to remain unchecked?
