What is the best activity to use for the attached exemplar document? The idea is that learners are able to identify and negotiate targets. It needs to be learner driven, but with the opportunity for tutors to have input/give feedback and advice. Thanks in advance.

the Choice might be an appropriate one, but it doesn't allow teacher comments, so you should combine it with messaging, for instance, and you cannot have the table format. The same for Feedback.

Sounds like something you could get working with Competencies and Learning Plans, combined with course and activity competencies.
You could do the following:
Create a set of activities in a course for a specific skill like the targets you listed in the document
Create a self assessment quiz at the end of this set of activities linked to the competency / competencies covered
Set the competency / competencies to be marked as 'competent' when the activity is successfully completed
In addition, you could add these competencies to a Learning Plan for each student, so they have a progress tracker where they can see how they're doing and where to go next to practice, learn and succeed.
