I am not sure exactly what you are looking for, but I will offer an idea.

If all you want is an honesty statement, create a quiz question using the Description type and then make this be the first question of every quiz. I will provide an example of this technique.

Or...create a 'question' at the site level and require all teachers to add that to their quizzes...It could be a description type.

Joseph...That works so well...

Thank youfor the offer, but I need something that would work by the default cause teacher forget to add custom text, so they would forget to add the question as well.

Oh, I think you need the teacher orientation quiz to have this question:

"I am a capable teacher, and I will not forget to put important questions into quizzes. If I do forget, I understand that I will suffer the consequences.  True/False." 😁

If you want this to be a standardized site wide quiz policy statement, a great solution is Acknowledge plagiarism statement access rule. You can use language customization to fit the text to your site.

Thank you for your answer, but we need to use custom text. As far as I know the Acknowledge plagiarism statement access rule

has it's own text that can only be translated, but can not be changed to any custom. What we do now os that we add our text toquiz description and turnon the access rule. But teachers turn to forget to use one or another.

As Daniel said, you can customise the text in the standard way that lets you customise any text in any Moodle plugin. Here are the docs: https://docs.moodle.org/en/Language_customisation
