I have searched for an answer to this but not found one.

Firstly please note this is not happening because people are attempting to hand edit html and copying code back and forth etc.

We've had an instance Moodle 3 installed for about a month now.  All seemed fine at first but within a week or users were noticing that images and links could be inserted with the text editor and seem OK but on saving the link became broken.

Trying this myself I get exactly the same thing. 

Turning debugging on no messages appear while I am using the editor (including when I click save).

In the statusbar I saw ...brokenfile.php...

This made me look into the source in the editor at each stage.

It goes from draft to broken and never gets to pluginfile.

I have purged the caches with no effect on this issue. 

Something is corrupt in the database I think. 

Does anyone know a likely way to fix this? 

Fortunately this is not the main teaching VLE.  It is used for staff resources.

This is also happening when the user drags and drops an image into a page.
