when a teacher add his/her comment, want to upload a file (recording voice) to the comment. Is this possible? if yes, please guide me through it.

Interesting idea, for sure, and one that is coming into its time, but I don't think it is available yet. The real issue would be ensuring that comments are directed to the person getting them, and not to anyone else. Unless someone else has a better idea? You might want to start looking for something suitable in the Plugins Directory. There may be something there that might be adaptable to meet your need, if you can discuss it with the maintainer of the plugin, or perhaps a new plugin type. Yep.. good idea for a new capability for Moodle.
Actually, I have poodLL plugin available, which allows voice recording. But I don't know how can I make quiz comment uses poodLL.
I don't believe that quizzes work with Poodll Feedabck in that way, but consider making an Assignment (called "Quiz Feedback" that students cannot submit. Then you can use the Poodle Feedback back feature here to record your feedback for each student. This will appear in their gradebook too (I think you would need to mark it as extra credit though) to not mess up your course points total.
Thank John. This is a neat idea. However, I need the feedback after each question in the quiz.
I guess for now, I have to persuade my teachers to Write their feedback.
