We are using Moodle 2.1.12. In one course, the grader report is as shown in the attachment. All grades are replaced with the word Error. However, grades are there and they download into a spreadsheet perfectly. The teacher of this course has added and deleted a number of items, some of which did have grades associated with them.
Any help would be greatly appreciated.

Can you post the categories and items page? I would imagine this has something to do with deleting items from the categories and items page but not deleting the activity....
For some reason at times the needsupdate field in grade_items gets set and won't unset (probably hitting an error someplace). Run this query:
UPDATE mdl_grade_items SET needsupdate = 0
WHERE courseid = <COURSE ID FOR THE TROUBLED COURSE>
Then refresh the screen
