Fullscreen Editor disabled and can't find where to enable it.

Running install 1.9.2 (20080924) and can enable full screen edit mode in the HTML editor. I can't get the button to appear in both FireFox 3.5 and IE7,8.

I have looked under Appearance - HTML editor and it says that it's enabled, but the button is not even showing up on the tool bar (seen screen shot).

Any place else I need to be looking to turn on this function?
