I know I had an issue with something similar and it was related to the case of the profile field - I had changed the case (capitalized it I believe) and it would do this again and again. I felt like there was an upgrade way back when that fixed this but I also remember having to do something with the profile field to get rid of this screen. Sorry, not a huge help but hopefully it will at least get you in the right direction.

Is that a student grade you are showing the 0 for? Because if it is your grade, it will not record a grade because you have the ability to preview the quiz.
You are using a gradebook - anything that has a grade in moodle goes to the gradebook.
Log in with a student account (don't just switch role, that won't help) and see if it still does the same thing.

We would need to see the quiz settings and the grade settings...also, if it is in a scorm activity, then we would need to see the scorm settings...
