Just popping in to say that it works as intended for me using Firefox (and Chrome) if you do indeed use the "HTML editor with file picker" format.

Where are you seeing the category name with the course? Do you have a custom theme?

Try as administrator to go to Site admin > Plugins > Blocks >Course overview and uncheck the box "Display categories"

Where is this image supposed to display? In a Moodle page or label? As an uploaded file? When you say you select to create a link to a file, what have you been doing - are you referring to an image uploaded to your Private files for example? Some more details could be helpful
