As in the subject title -the ticks are there when changing to, for example, Clean theme. It is Moodle 2.5 and is there a simple way to make them visible in the custom theme by adding a bit of code somewhere -or is it more complicated than that? Thanks.

Joseph -from my old schools's point of view -they used guest questionnaires extensively for parents and other interested parties for surveys on for example, school uniform or school dinners (very British) Also there is a group of pupils who embark on a Business Enterprise course and they would use a questionnaire to gauge interest - and later feedback - in certain products they were planning to market and sell. Also the school used questionnaires as a sign up form for parents who wanted to come on free evening classes for example. Most times these questionnaires were in other courses (not the front page) and the courses had guest access, and the questionnaires also had guest access. However, I think if guest access to questionnaires in courses is not possible or too difficult to implement then I don't think it is that arduous to add it to the front page. As for one guest completing them multiple times, I hadn't thought of that - we only expected them to complete it once.
The questions varied - usually choosing from a dropdown, or rating 1-5 or typing in some text. And yes, I can see some questionnaires being completed by logged in users as well as guests.

You are correct in that it is no longer possible to export a wiki- -please see /MDL-24439
No you can't. There is a workaround but use at your own risk - this is for your version of Moodle - look at #12 http://docs.moodle.org/24/en/Course_FAQ
Good post Derek. I agree with all of those, although from personal experience I most favour the two image related ones and the groups in forums one. Anyway, I have voted.
