The previous "wrong PHP theory" is wrong. There are other Moodle instances running on the same server with zero problems. I checked H5P (black icon) just now by creating an activity.

Nobody has an idea? If more information is needed, please ask!

Made an observation that supports a PHP mismatch. After the production 4.1 LTS was upgraded to 4.5 LTS (following the last test upgrade we did in the staging server) I made a new testing instance by cloning the production (now on 4.5 LTS) in the staging server.

This new testing instance does not have the said problem!

The main difference between the two servers is in PHP: The prod. is on 8.1 whereas the staging server is on 8.2. I know, this could be a red herring, but I am looking for ideas.

A default config? Again! Ref. H5P content types cannot be downloaded. 

 

Moodle does not come with a (default) config.php.

It is possible that what OP needs is, when an alumni of that quiz go to review of an old attempt, the new feedback will appear. I haven't tested but according to Matthias Regrade all will make that happen.

Isn't it what it says? You have tried a backup of the same course which is not yet finished - or crashed. Moodle says, no more copies until that matter is settled?
