Limesurvey est un excellent programme de sondage, open source. Très complet, mais je trouve son interface inutilement compliquée par endroits (types de questions, options, etc.).

Are you getting this error with all of your questionnaire instances or just one of them? Have you tried with a very simple questionnaire including just one MCQ question? Has this problem developed recently?

That text is centered. See attached screenshot, using standard theme.
This is probably a theme issue. Which theme are you using? straight from install or customized? Which brower, which version?
