I'm not an expert on this question type but could it be because you have the "cm" included in the submitted answer? What happens if you just enter 2.34 as your submission?

Building upon Tim's response, I made a on how you would enter a calculated formula using the formula you provided.
From your image I can't even tell which item is locked (maybe my eyes are already going bad at 24 years old ). Also you didn't indicate which assignment was having the issues.
I'll take a wild guess at the issue though even without the above mentioned info. This could be an instance of browser cache affecting instructor's view. Have they tried clearing their cache and the grades still aren't there?
Moodle exports the gradebook alphabetically by last name. If you did some a sort in Moodle, this won't be reflected in the exported file. You could always just re-sort using Excel's sort function.
The information column usually displays where the action is taking place. To reference your screenshot, the third row has an action of Course view and the information column has Moodle Features Demo. The Course view action is a log referencing that the user (which appears in Full Name column) was on the course homepage. Moodle Features Demo is just referencing the name of the course.
The numbers though are a little harder to explain. Since Moodle is open-source, anyone can write a plugin. With anyone being able to write their own plugin, there may be some loss of consistency as to what shows up in the reports. I am not familiar with the Hotpot module so I can't say exactly what that specific action is.
