A "use grades" flag is a good idea ... you should file a feature request on http://moodle.org/bugs

Yes, to avoid this you must use the 'save grades' button on the quiz editing page. Even on an existing quiz you can go back and do this, then visit the quiz report and "regrade" to fix everything.

The little bug causing this has been already been fixed in the next version.

Usually, a double quote (") gets converted to an escaped version (\\\") when the file is being saved by the web-based language editor. You never see it in the web interface though. At least that's how it is supposed to work!

I see the bug (it only occurs if you have debug turned on), and I've just fixed it in CVS.
