Have you set the correct permissions. Check that you have the correct read/write permission...CHMOD 644?

Thank you for your reply, but as I can see, the file "block_site_main_menu.php" does not even exist, I have installed the latest version of moodle, and I was checking if the problems is that I forgtot to uploaded, but this is not the cassse..please advise ..
Which version (exactly) did you download, and where from?
To solve the problem just delete the empty folder blocks/site_main_menu

I just got an identical error downloading version 1.3.1.zip (easynews mirror I think) (today 6-15-04) to an apple XSERVE. The file was unzipped into a new folder and installed over an existing 1.2 database.

Just for your information, the folder is still included in the present version 1.3.2 downloaded from moodle.org.
Deleting the folder removes the error message.
