We have a dedicated server with moodle and the following config:
Moodle 4.1.1+ (Build: 20230203)
PHP 7.4.33
MariaDB 10.6.14
Very randomly a user is doing a course with scorm and gets stuck in a middle interaction/question with no solution to go back or move forward, only a blank (grey in this case) screen and the option to submit but not to choose the response.
The only chance to go forward is to reset the course for this user that needs to do the entire course again.

In the server and moodle logs we cannot find anything related. We have also create a develop environment and do some changes in the server and moodle config but the issue happens randomly and we couldn't replicate the behavior at this moment.
In attach I have included two imagens as an example of the error and what other users see.
Any idea what could be related?
