Not sure why I got this error notification during the upgrade process...  I'm really wondering if it's a false positive - opache is installed on the server and seems to be running correctly.

Is there somewhere I should look to investigate this issue?

AFAIK "missing from disk" it's a Moodle message, so you should check if a Moodle "opcache" plugin is missing, maybe you installed this: https://moodle.org/plugins/tool_opcache and in the upgraded moodle this plugin is missing ?
The message you got should not be about the server's opcache status.

Hi Sergio,
Yes, I did install that tool_opcache plugin, but uninstalled it a while back. The file structure wasn't part of the upgrade so this must be something residual in the db.
Thanks for the help.

You should remove the plugin registration from moodle's plugin list, where you should be able to request the plugin uninstall.
