Moodle 3.9+
When I try to add an assignment or edit an existing assignment I get a http 500 error.
Here is an example of a link: http://xxx.xxxxxx.org/course/modedit.php?update=123&return=1

Found the problem myself.
Annotate PDF Advanced was the issue. Removed the plugin and all was good.
Anyone else running into this issue, I would suggest removing plugins you've install (not default ones) one at a time checking after each until you find the problem or you're out of plugins.
