We are using Moodle 3.9. Teacher has reported this issue, I have tried to add an assignment myself using my own admin account and I get the same error.
How do I troubleshoot this?
The disk nor the database are full.

I have more teachers reporting this issue now. I don't even know where to begin looking, or what possible workarounds I can suggest.

We have a workaround, that is to use "Save and return". When using "Save and display" the issue occurs.

Raise the https://docs.moodle.org/39/en/Debugging level to its maximum and get a trace when it happens.
