There is a general method for using a grading form with a quiz (or any other activity).
- Create the quiz and set the total point value to zero so that it does not appear in the gradebook.
- Create a assignment next to the quiz with the grading form attached to it. No submission type is set. The visibility of this can be set to stealth mode so that it is not seen by the student on the course page, but appears in the grade book.
- The teacher waits for the student to complete the quiz and then enters appropriate values in the grading form on the assignment.
- A link from the quiz description or feedback to the assignment feedback can be set. This is easily done with autolink filter and activity completion to show after attempt is submitted or the form is graded.
There are limitations. The teachers need to calculated any subscores themselves. The method is not linked to attempts so teachers need to work out how to deal with possibilities of multiple attempts.
