I m trying to get the following report for all courses in our moodle site, can someone please tell me which SQL tables should I looking at. It is bit hard to get activities for all courses like this, that's why I m thinking of SQL.

As far as I know you will need to look into the below tables.
course,course_completion,mdl_grade_grades,mdl_grade_items.
