I would like to provide users with a post-course action plan where they identify a number of things they will do differently following the course along with a few other fields. I need to be able to allow the course trainer to moderate the entries to ensure that they are relevant to the course learning objectives and then allow the delegates line manager to see the entries and to "sign off" on completion. I also need to be able to see all actions and completion status course wide for the trainers and topic for the overall administrator/programme director.

Any ideas on how this can be done using databases?
