When I preview a question, I'm not seeing the changes I made--the preview is not accurate. When I log in as a student, the changes are not shown as well. Anyone experience this strange behavior before?
Are you sure that you are making the changes to that specific question and are not saving as new question?
Yes, I just checked the question bank and I'm not accidentally duplicating

Do you have more than one web server in your system? If so, is caching configured properly?
Our Moodle account is hosted by an outside vendor. Maybe I should ask them
