The TinyMCE editor has a way of creating 'templates' that I have used for this kind of thing previously.

Another option might be to consider using the Dataform activity instead of a forum.

Have you installed the plugin Generico (works with Atto editor) at your site? You can create your own templates and add them wherever needed.

If you don't need to allow replies, the Feedback tool or Questionnaire plugin might serve your purpose better.
