We have one category where one teacher's questions are seen in some other courses.
He is not admin.
What could be the reason?
In this server we don't have a "shared question" role?

You don't necessarily need a special role for this to happen. It could also depend on what permissions your teachers already have or where this teacher saved their questions when he/she created them.
When the questions were created, what category were they saved under: the Quiz/Course category, or at the Category/System level? And do your teachers have the permission to view all questions or just their own?

Actually we have one teacher who have questions in one course but those questions appear for other teachers in their courses.
So, what might be the cause?
We don't have shared question role in this site.
