Passwords Being Automatically Added to Quizzes

After posting new quizzes, students contact us letting us know that they need a password to take a quiz--yet we set no passwords on them.  Passwords are, somehow, being added automatically under "Extra Restrictions on Attempts" and we need to manually remove them.

Any idea how these password are being added and how we can prevent it?

It is probably an over keen setting in the browser. See if the password is the same as the person setting the quiz.

Your browser is automatically recognizing the field as a password field and filling your username password in there.
