Add small non-self-marking text input box in a quiz

I am putting together a language placement test using version 2.6.  The test will be taken on campus by people who may or may not yet be a student at the university.  Therefore, instead of linking to the LDAP or creating individual accounts for each person who registers to take the test, I want to create a list of dummy users (e.g. TestTaker01, TestTaker02...etc.) and then assign them to students as they register.  But if I do this, I would also like to add a text input box in the test that will allow students to enter their full name and contact information in the quiz.  The only "Quiz" function I can see that does this is "Essay" but the box is way too big.  So...three questions:

  • Is there a way to insert smaller non-self-marking input box(es)?
  • Is there a registration add-on that might manage the entire process?
  • Would anyone recommend a different approach to this project?

Use a short-answer question. Set it to be worth 0 marks in the quiz. Set it so that the right answer is '*' (which means match anything).
