I installed database in my moodle 2.5 but i can't add entry.
I created the views and the fields, but i don't see "Add entry"

I resolved this problem, but now i have other problem. I add a new entry and save the changes I get a read error on database.

We need more details on the instance set up to be able to investigate. Please open a tracker issue at https://tracker.moodle.org/browse/CONTRIB/component/12338 and indicate Dataform version and a scenario for reproducing the problem.
