I would like to create a form in a custom moodle folder (something like moodle/form).
Reading the documentation I made this two file (it’s a very early version) but I can’t see the editor.
Could you help me, please?

Nothing jumps out at me, but do you have Developer level debugging switched on?
Just an aside - always write Moodle custom code as proper plugins (e.g. /local/myplugin, /report/myreport) rather than creating a whole new structure. Loads of stuff is done for you
