Hi Tim! Thanks for the prompt response. We actually upgraded from 1.9.5 to 2.2.3 and it's the first time we've experience this. I went to check how the questions behave thru Admin - Plugins-Questions behaviour-Manage question behaviour-manual grade. It shows that Manual grade is enabled.

Please advise when this will be resolved and what steps we need to take moving forward.

Having manual grading enabled there is OK (but dangerous).

What you need to check is, in the specific quiz Edit settings -> How questions behave.

Tim, i realized that i didnt answer your question. No, i did not set the "how questions behave" to manual grading in the quiz setting. It is  set for "immediate feedback".

Also, you mentioned that manual grading enables in the site admin is ok but dangerous. How so?  

Do you have additional inputs on how i can go around this?

In that case, my initial guess about what was wrong was incorrect. However, at the moment, I cannot think of any other reason why Moodle would not have graded those mulitple choice questions.

Having that option enabled is dangerous because people might select it without realising the implications (that it prevents Moodle from automatically grading your multiple choice questions).
