Questionnaire (plugin)

Hi

I am using Moodle 1.9.11.

I am trying to create a question that asks users to score their top 5 priorities with 1 = most important and 5 = 5th most important.  There are 14 priorities in the list.  A user would score 5 of them and leave the rest blank.

I've tried to do this with the Rate 1 .. 5 question type, but that doesn't seem to do exactly what is required.

Is there a question type that I can use?  Is this possible in Moodle 1.9?
