I want to avoid building a course applciation form outside Moodle. Which is the best Moodle tool to use for this? I have tried the feedback module but it doesn't allow file uploads.

Did you manage to work this out? I've been asked to create a Application for Extension form to be filled out and submitted on moodle. Students however may need to upload a medical certificate. I too looked at using the feedback module but as you found you can't upload a file. Any ideas?

You could use the Database activity module and set entries to be moderated before appearing. This means that only the teacher will see them but no other users will see them - so the application forms are private but file uploads are allowed.
Mary is right, you can use the database, but there is a bit of an element of lack of coolness with this.

This allows tutor <> participant private communication.
