I working on project where i have requriment "to find the user id who have created questionnaire".  Please help me out. i am searching for a while but could not found.

I suppose the person who wants to know the identity of the person who created a questionnaire is an admin on your moodle site?

Here is a scenario, if you know the name of the Questionnaire and you want to know which person (teacher) created that questionnaire in a course.

Go to your Course Admin Report page and request information similar to attached screenshot (made with Moodle 1.9).
