I'm running Moodle 1.9 (The exact version it gives me is Moodle 1.9.9+ (Build: 20100707)) for a business' training department.
We had a teacher show us two grade reports. One showed completed assignments, and the other showed missing assignments. I verified that the grades were missing, and we're hesitant to restore from a backup until we know the root cause of this.
Does anyone have any idea what went wrong?

When you say assignments, do you mean that instructor used the Assignment tool to provide grades?
So, are the grades appering in the Asignment area, but not in the gradebook, or are they not appearing anywhere?
Screenshots would also help.
The grades were generated with built in quizzes.
The instructor pulled a grade report, and later pulled another one, and the latter one lacked grades that were on the original report. The grades and the attempts don't even show up in the database.
The instance we use is for internal corporate training, I'm not sure if I can get screen shots, I'll see if I can get it cleared.
This does not sound like a gradebookj issue, if your quiz results are now gone from the Quiz interface.
If I understood you corerctly:
- There was one course where students took a quiz. Then instructor printed the gradebook that included quiz results.
-Instrucotr came back into the same course after some period of time, only to find that student quiz attempts are now gone?
It sounds like someone deleted the quiz results via the quiz interface, if they are not displayed in the quiz anymore (which would cause them to disappear from the gradebook as well)
Or, in case you had some upgrades/code changes during your "semester" - that could also influence things.
