On category and course creation: "A required parameter (courseid) was missing"

I found in this forum that this problem exists as mentioned in another post, however that post was originally dated 2008. I did not see any direct resolution to the issue. I wanted to open a new discussion to see if anyone has a solution to this problem.

When I create a new question or create a new category and save the question I receive an error message stating "A required parameter (courseid) was missing".

When I select the "continue" button I am kicked out of the course and back to the Moodle main page.

I have to enter the course again and navigate my way back into the question bank to enter a new question or to work with the one I was just on.

This is not consistent as sometimes it will allow for a category to be created, sometimes not. It is however consistent when creating a new question.

I am running:

Moodle 1.9.9 (Build 20100609)
mySQL 5.0.67
php 5.2.8
apache 2.2.11

Can you find a sequence of actions that reliably reproduce this problem? Obviously, most people can create questions most of the time with no problems, or we would have heard howls of anguish here. So we need to work out exactly what triggers this error in your case, to pin down the bug.
