I created a dozen on line quizzes for my class this semester. Most contain one or more questions that load images and all are in HTML format. Each was also verified by two of my grad students before the semester started.
All semester, I've had students report that images are not being displayed for some questions, following no pattern except most report that Moodle auto-formatting was use (I used HTML in every case.). I just had some students report missing images which I fixed, only to have others vanish.
Oh, when I check the questions, each does show one pattern. In each case, I get:

There is no closing and link to the image has vanished.
