Is there a way to allow students to leave comments in a quiz?
Let's say a student is doing a quiz and they have a question, they could click a button right in the quiz, and leave a question? Then the teacher would get a notification indicating the question and what quiz it was?

No. They would have to go to the normal Moodle messaging system to send a message to their teacher.

As Tim advises, this is not possible however I think if you provide an Essay question for 'General comments' the students could write this sort of feedback in one go. The teacher can then view this feedback via the Manual Grading area of the quiz results.
