I am just wondering if this is more of an issue with Grade sych than the quiz. I think I will move this to the LTI forum. (If that was wrong, it can be moved back.)

And, from Moodle 4.4 onwards, the feature is available in the standard Moodle package.

I think the way to work out what is going on is to read through the chunks of lib/db/upgrade.php which mention question_set_references that. That will show what the upgrade thoguht it was doing.
The other main source of information is the 'caused a regression' links on MDL-72321. Is your issue already reported there?
