Short answer: Sourceforge CVS sucks.
If you don't need to commit changes (or don't need to commit changes very often) you will almost be better off using one of the mirrors: http://docs.moodle.org/en/CVS_for_Administrators#CVS_Servers via anonymous login.
It is only really core Moodle developers that need to suffer the Sourceforge CVS slowness - an if more people start using the read-only mirrors, then it may even make Sourceforge faster for those of us stuck there
Longer term, I think Martin is thinking of moving to his own CVS server, but I think the 1.9 release comes first.

This is a long-running problem that is cause, at least in part, by Excel being not very good an handling Unicode files. See issues MDL-7993 and MDL-9911 for some of the background.
I get the impression that Eloy has not done just about everything possible to make this work as well as it can.
So, hopefully, if you can upgrade to 1.8.2+, it should work.

On your quiz settings page, you have Adaptive mode set you 'Yes', and you want it set to 'No'.
