Ask them if they have cleared caches following the upgrade. Did the question banks go through the upgrade process without error?

There is obviously something about those three students that is causing them not to show up there - look at their enrollments, look at their roles in the course, look at their groups if you are using groups...

You should be fine then. To be safe, download copies of the following - the moodle code folder, the moodledata folder (clear out the cache and temp folders prior to the copy just in moodledata folder to minimize size - they get recreated) and an export of your database. With those three items completely copied, you can recreate your site anywhere if need be.
I have a permissions issue. One of the sites I work on has a trainer role and that role has to be able to see all the certificates for the users in its group. This is working just fine. The problem is that they can also click on View Certificate and create a certificate for themselves whenever they want - is there a way to set it up where they can see the certificates their group users have earned without being able to create one for themselves? (They should be able to get a certificate but only when all the requirements have been met.) Have I set up the permissions wrong or is this not possible?

And did you start the exe from the directory as it states there?
