It tells you what the problem is - I am not sure what more we can do, except tell you to check the database connection details...

The best way to test is to log in as a test student and as a teacher - Do not switch role - actually log in as a user with each role...and just to clarify - when you set up the course, you selected Relative Dates on this screen?

 

After you create the assignment, you should see this:

I am pretty sure that the course has to be originally set up for relative grading - at creation, not after that fact...

It is clearly pointing to the mathjax filter - if there is an upgrade, upgrade the plugin. If not, disable it.
