mod_assignment(Assignment 2.2) is the name of the old assignment activity which was disabled by default in 2012 and removed completely in Moodle 4.2. See the section Assignment 2.2 is removed in the 4.2 docs. The current assignment is named mod_assign.

This is when creating a course, yes? What theme are you using? If it is not a standard theme, can you change to Boost and try again? I don't see the same icons as you when I try to add a course image.
