Just a thought... Why not simply create the accounts manually? Is that an option for you? More info here - http://docs.moodle.org/en/Manual_accounts_only
Go to http://yoursite/moodle/admin/auth.php to find the authentication options on your site.

Also: Have you tried changing themes? (This was a solution to a similar problem for me.)

One way to do this would be to simply add an "offline assignment" and give each student a grade for it.
Maybe someone else will jump in here with a more sophisticated solution.
